How It Works

1. Create a Spravato Treatment Note

From the patient chart, create a new charting note and select the Spravato Treatment Note template. The note includes two sections:

  • REMS submission fields — maps directly onto the REMS Patient Monitoring Form (patient info, provider info, treatment date, dose, lot number, monitoring duration, concomitant medications, pulse oximetry checks, and SAE reporting)

  • Clinical documentation fields — assessments, medications, allergies, and other standard charting fields

First visit: All REMS fields will be blank. Enter patient name, healthcare setting information, and provider information to populate the PDF correctly.

Subsequent visits: The note pre-fills from the previous session. Review each section and update any session-specific details that have changed — lot number, vitals, and yes/no monitoring questions in particular.

2. Complete all required fields

The note cannot be signed and locked until all required REMS fields are filled in. Two additional fields are also required before signing:

  • Diagnosis (pulled from the patient chart)

  • CPT code (entered manually)

A banner at the top and bottom of the note reminds providers: "This note must be signed and locked to submit the REMS Patient Monitoring Form via fax. Submission is required within 7 days of the treatment session to remain in REMS compliance."

3. Sign and Lock

Select Sign and Lock to trigger submission. Locking without signing will not submit the form.

Any authorized user with charting permissions can fill out and sign the note — it does not need to be the rendering provider. Only one signature is required.

4. Confirmation

Submission takes approximately 4–5 minutes. Once complete:

  • An addendum is added to the charting note confirming the fax was sent, with a timestamp

  • The submitted PDF is saved to the patient's Client Documents under a "REMS Monitoring Forms" folder, where it can be viewed or downloaded at any time


Troubleshooting

Note was locked but not signed
The form will not be submitted. You'll receive a task notification with instructions to unlock the note and select Sign and Lock. If you don't have access or the necessary permissions, please reach out to our support team.

Required fields were incomplete
The system prevents signing and locking until all required fields are filled. If a backend error occurs after locking, a task notification will outline the issue and steps to resolve it.


Common Questions

What if I need to make changes after the form has already been submitted?
Once faxed, the note stays locked and the form cannot be re-faxed automatically. You can add an addendum to the charting note for documentation purposes. To resubmit the REMS form itself, do so manually at SPRAVATOrems.com or by fax.

Can my nurse fill out the form and I sign it later?
Yes. Any authorized user can complete the form. Only one signature is needed to trigger submission.

Where can I confirm the form was submitted?
Two places: (1) the addendum on the charting note with the submission timestamp, and (2) the PDF in the patient's Client Documents under "REMS Monitoring Forms."

What if I accidentally lock the note without signing?
You'll receive a task notification with instructions. Unlock the note and select Sign and Lock to complete submission.
